New Patient Registration
We will register any patient living in the IG10 postcode area dependent on capacity. New patients must first complete a registration form available from reception. One you have completed the paperwork please return the pack to reception between 9am and 4pm Tuesday to Thursday so that receptionist will be able available to photocopy your documents and check through your forms. If you are aged over 16 or over you will also be asked to use our health check machine for blood pressure check and BMI measurement, you may also be asked to attend a new patient medical check once registration is completed.
Your registration will be with the practice, not with an individual doctor, though should you express a preference we will endeavour to meet your needs.
Medical treatment is available from the date your registration is approved this may take up to 14 to 28 days .
