Nurse Practitioners

Leona (RGN, Nurse and GP Trainer) is our Nurse Practitioner and Prescriber with speciality in Long Term conditions, Leona trained as an ICU Nurse at Oldchurch Hospital in 1987 and joined Forest Practice in 1994. Leona is responsible for running the majority of Chronic Disease Clinics including Diabetes and Multi-condition reviews.

Nurses

Gail (RGN, Nurse Prescriber) and Nicci (RGN, Nurse Prescriber) Practice nurses are qualified and registered nurses. They can help with health issues such as family planning, healthy living advice, blood pressure checks and dressings. The practice nurses run clinics for long-term health conditions such as asthma or diabetes, minor ailment clinics and carry out cervical smears.

Healthcare Assistants

Jacqui is our HCA. Healthcare assistants support practice nurses with their daily work and carry out tasks such as phlebotomy (drawing blood), blood pressure measurement and new patient checks. They may act as a chaperone when a patient or doctor requests one.

Practice Management

The Operations Manager Louisa Adams is involved in managing all of the business aspects of the Practice such as making sure that the right systems are in place to provide a high quality of patient care, human resources, finance, patient safety, premises and equipment and information technology. The Operations Manager supports the GPs and other medical professionals with delivering patient services and also helps to develop extended services to enhance patient care.

Administration

Dawn (Admin Manager), Claire, Corrine, Sue and Taylor. Our Administration team provide administrative support to the practice. Including new patient registrations, booking post-natal appointments for mother and new born baby, medical correspondence and insurance requests.

Reception

Sue, Rebecca, Melissa, Joanne, Claire, Renee, Lisa, Rachel, Julia and Baljit. Receptionists provide an important link for patients with the practice and are your initial contact point for general enquiries. They can provide basic information on services and results and direct you to the right person depending on your health issue or query. Receptionists make most of the patient appointments with the GPs and nurses. They also perform other important tasks such as issuing repeat prescriptions and dealing with prescription enquiries, dealing with financial claims, dealing with patient records and carrying out searches and practice audits.

Secretaries

Denise & Amanda. Our secretaries provide the secretarial support to the practice.

First Contact Practitioner

The Practice now has the services of a 'First Contact Practitioner' twice a week, whose role in primary care is to access patients with soft tissue, muscle and joint pain and to decide on the most appropriate management pathway. FCP's are practitioners with expertise in the assessment and management of Musculoskeletal(MSK) conditions. They can organise investigations, medications and onward services. Reception may direct you to them if appropriate.

Social Prescribing Link Worker

We now have a Social Prescribing Link Working at the Practice. The main aim of their role is to help people to improve their health and well-being by connecting them to activities in the community. Line Workers connect community and activity groups to work, debt or housing advice.
